After Hours & Emergencies

If you have an urgent matter after hours and need to speak to the physician on call, please dial 801-569-1999. You will be given the phone number of the physician on call.

If you experience a life-threatening emergency, PLEASE DIAL 9-1-1 IMMEDIATELY.

Referrals

We will be happy to assist you with a referral to a provider that your primary care physician has recommended. If a referral is necessary, we request notification at least 72 hours in advance of your appointment. Same day referrals will be processed for urgent appointments.

Prescription Refills

Prescription refills are best handled at your regularly scheduled office visit. During this time, we are able to review the proper dosages, as well as address any questions or concerns you may have. If it becomes necessary to refill your medications between appointments, please allow at least 24 hours for prescriptions to be phoned to pharmacies and at least 48 hours to process requests for written prescriptions.

Chronic or long-term medication refills require an office visit before they may be refilled.

Obtaining Your Medical Records

A medical release waiver must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider. If you are requesting that your records be sent to another provider we ask that you supply us with their name, address, and phone number.
